Encounter Hooks & Tactics
- A trusted NPC ally begins acting strangely — forgetting names, making odd decisions, insisting on detours to isolated locations. An intellect devourer consumed their brain days ago and is piloting the body. The party has to figure out something is wrong before the devourer leads them into a mind flayer ambush.
- The party finds a survivor in a dungeon who begs to be rescued. They seem confused and disoriented — classic signs of trauma. In reality, their brain was devoured and replaced. The intellect devourer is gathering intelligence on the party's capabilities for its mind flayer masters.
- An intellect devourer has taken over a city guard captain. It's been selectively ignoring crimes in one district, allowing a mind flayer cell to operate undetected. The party is hired to investigate the crime wave, but the trail leads to the captain — and the thing wearing his body.
Combat Tactics
The intellect devourer is one of the most terrifying CR 2 creatures in the game because Devour Intellect (DC 12 INT save) can instantly reduce a character's Intelligence to 0, stunning them. Then Body Thief takes over. Target fighters and barbarians — they usually dump INT. The devourer is Tiny and has Detect Sentience at 300 feet, so it knows exactly who to target before combat starts. Run them as ambush predators: they hide inside corpses or in dark crevices, wait for the party to pass, then strike the backline character with the lowest INT. In a mind flayer lair, scatter 3-4 of these ahead of the main fight. They're disposable scouts and assassins, not standalone threats.
Works Well With
Intellect devourers are created by mind flayers and serve as their forward scouts. A mind flayer lair should have 2-4 devourers roaming the perimeter. The devourers soften targets before the mind flayer closes in with Mind Blast.
Umber hulks and intellect devourers are both Underdark ambush predators. The umber hulk's Confusing Gaze disorients the party while the devourers target anyone stunned or separated by the confusion.
Aboleths and mind flayers compete for psychic dominance in the Underdark. An intellect devourer found near an aboleth's lair suggests a mind flayer incursion — the party stumbles into a war between two psychic horrors.
